Tennessee's attempt to execute Christa Pike by pentobarbital fails; she survives two doses; Gov. Lee pauses 2026 executions and orders a third-party review

On Wednesday 2026-09-30 (event date) Tennessee tried to execute Christa Pike with the state’s one-drug pentobarbital protocol. A Sixth Circuit stay delayed the 10 a.m. CT schedule; the U.S. Supreme Court lifted it about 6 p.m. (6-3 per TIME). Injection began about 7:26 p.m.; curtains closed and reopened several times and witnesses were told to leave at 8:53 p.m. Pike received two doses and survived; her attorneys said at least seven needle sticks pierced her left arm, her right arm turned purple, and the drugs may have entered tissue rather than the bloodstream (attorney characterization, not a state finding). The Department of Correction said it “followed every step” of the protocol. Late that night Gov. Bill Lee paused executions through 2026 and ordered a third-party review; on 2026-10-01 he said “something didn’t go well or right last night.” Pike was reported in critical condition in a Nashville hospital as of 2026-10-01 (attorneys’ account). DPIC calls it the first modern-era case of a person surviving lethal-injection drugs; that framing is DPIC’s.

Why it is in the Hasan file: Doug Wilson told the Guardian (published 2026-10-15) that he prefers a firing squad to “attempts to ‘medicalize’ it, as with the recent botched execution by injection in Tennessee.” Wilson does not name the case. Pike is the only Tennessee attempt of the last month found; an earlier 2026 Tennessee failure (Tony Carruthers, reported 2026-05-21, IV access problems, one-year reprieve) appears in a DPIC headline and search summary but was not read here, so the identification with Pike is an inference. The Hasan sequence is in the 2026-10-05, 10-08 and 10-15 entries.

Gaps: Tennessee Department of Correction’s own statement and Gov. Lee’s order (Tier-1) were not retrieved; CNN’s 2026-10-05 piece on the outgoing corrections commissioner returned 451. Pike’s underlying conviction is not described in the sources read and is deliberately left out.
