Acentra's ICE Medical Claims System Has Processed Zero Claims While Hospitals Absorb Detainee Care Costs

ICE has paid no third-party medical provider for detainee care since October 3, 2025, when the Department of Veterans Affairs abruptly ended its role processing detainee medical claims. The replacement is a contract with Acentra Health, valued at up to $67.5 million, to stand up a new claims-processing system. As of the reporting window, Acentra has processed zero claims.

The failure is documented at both ends of the contract. Acentra had received $44.6 million of the $67.5 million as of June 4, 2026, against a contract term set to end July 31. Processing was to begin April 30, 2026; when that date passed, payments to third-party providers did not resume, and as of August 25 the IHSC provider portal still described itself as “coming soon.” On the provider side, UCHealth is owed roughly $4 million for detainee admissions it has already absorbed.

Federal contracting documents themselves warned that payment delays could cause community providers to stop accepting patients in federal custody — the predicted consequence, recorded in advance, of a system that pays a contractor most of its money for a service it has not delivered. That consequence lands on a detention population whose medical outcomes are already the subject of a documented death record (2026-06-12–eighteenth-ice-detention-death-of-2026-denny-adan-gonzalez-deadliest-year is one marker of the trend). The structural point is not that care was denied by policy: it is that the payment rail was privatized, the privatized rail failed, and no mechanism forced either correction or refund while the contract ran its term.
