The DHS Inspector General published OIG-26-08 on June 2, 2026 (publication date), reporting on an unannounced inspection of Winn Correctional Center in Winnfield, Louisiana carried out March 4-6, 2025 (event dates), about fifteen months earlier. Winn holds ICE detainees under a Winn Parish Sheriff’s Office facility managed with LaSalle Corrections; ICE housed the contractual maximum of 1,576 men at the start of the visit.

OIG found Winn compliant for special management units, general hygiene and the grievance system, and non-compliant for environmental health and safety, food service, use of force, medical care, classification, the voluntary work program, legal access, staff-detainee communication, and SMU outdoor recreation. Facility staff used prohibited techniques in at least three of five use-of-force incidents reviewed and did not document prompt notification to ICE in four of five. On medical care, OIG’s contracted reviewers read 36 health records and found staff did not consistently update Master Problem Lists and treatment plans or document how lab results were interpreted. OIG also reported that facility staff gave incomplete responses and refused to hand over all video recordings. ICE concurred with all nine recommendations; four were resolved and closed and five resolved and open as of the report.

This is the “unannounced inspection” referenced in coverage of later Winn deaths: it is an OIG product, not an ICE Office of Detention Oversight report, and it preceded the death of Carlos Josué Marchena on September 18, 2026 (2026-09-17–marchena-dies-winn-correctional-25th-ice-custody-death-2026) by about three and a half months. The report does not mention diabetes or insulin and says nothing about any individual’s care; it documents record-keeping failures in the system a detainee with a chronic condition depends on.
